D904 referrals for podiatry

A plain-English guide to the DVA referral you need before your first podiatry visit: what it is, how to ask your GP for one and how long it lasts.

Written and reviewed by Daniel Dalirazar, podiatrist. Updated November 2026.

What is a D904 referral?

The D904 is the Department of Veterans' Affairs form GPs use to refer DVA card holders to allied health services such as podiatry. Your GP can also refer you with a letter on their letterhead, as long as it includes the details DVA requires. Either way, you need a referral before DVA will fund your podiatry.

Who needs one

  • Gold Card holders, for any podiatry that's clinically needed
  • White Card holders, for podiatry linked to an accepted condition

The Orange Card covers medicines only, so it can't be used for podiatry.

How to ask your GP

  1. At your next appointment, say: "I'd like a D904 referral to podiatry, please."
  2. Mention any foot problems you've noticed, such as thick nails, pain, numbness or slow-healing sores, so your GP can include them.
  3. Ask your GP to email the referral to info@themobilefootclinic.com.au, or keep a copy to give us at your first visit.
  4. Call us on 0493 367 109 to book your home visit.

How long a referral lasts

DVA funds allied health care in treatment cycles. A referral covers up to 12 podiatry sessions or one year, whichever comes first. Some consultation types don't count towards the 12 sessions.

At the end of each cycle we send your GP a report on your progress. If you still need care, your GP can refer you for another cycle, and there's no limit on how many cycles you can have. You can read more on DVA's treatment cycle page.

What happens next

Once we have your referral, we arrange a time to visit you at home. See what DVA podiatry at home covers.

Common questions

Ask us anything else on 0493 367 109.

Do I need a new referral every year?

You need a new referral at the end of each treatment cycle, which is after 12 sessions or one year, whichever comes first. We'll let you know when your cycle is ending so you can see your GP in time.

Can my GP use a letter instead of the D904 form?

Yes. A referral letter on the GP's letterhead is accepted if it includes the details DVA requires.

Does one referral cover physiotherapy as well?

No. Each type of allied health service needs its own referral, so ask your GP specifically for a referral to podiatry.

What if I've lost my referral?

Your GP's practice can send us a copy. With your permission, we can contact the practice for you.
