Patients we see
- DVA Gold Card holders needing podiatry that's clinically necessary
- White Card holders with an accepted condition affecting the feet or lower limbs
- Patients who find it hard to attend a clinic because of mobility, frailty, transport or cognitive issues
- People with diabetes or peripheral arterial disease who need regular foot surveillance
How to refer
Use the DVA D904 referral form, or a letter on your letterhead with the details DVA requires. Email it to info@themobilefootclinic.com.au and we'll contact the patient, or their family or carer, to arrange a home visit.
It helps if the referral includes:
- The patient's DVA file number and card type
- The reason for referral and any relevant diagnoses, such as diabetes or peripheral arterial disease
- Current medications, particularly anticoagulants
- The best contact for booking, if it isn't the patient
What you'll receive
Referrals are made under DVA's treatment cycle of up to 12 sessions or one year. At the end of each cycle we send you a report on the patient's progress and whether further treatment is recommended. We'll contact you sooner if we find anything that needs your attention, such as a new ulcer, signs of infection or worsening circulation.

Visits are provided by Daniel Dalirazar, an AHPRA-registered podiatrist with a Master of Podiatric Medicine from Western Sydney University and over seven years in practice.
